Prescription Refills
Please contact your pharmacy regarding your prescription refill. If you need further assistance contact the office.
Preparing for Your Appointment
We ask that you please check in online through our EZDerm patient portal powered by Sadio prior to your appointment. When you make an appointment, you should have been sent an email linking you to your secure patient portal on EZDerm. Please fill out all of your patient information, including allergies, medications, past medical history, family history, surgical history, social history and then sign all of the one time consent forms listed. Please have this completed at least 24 hours prior to your appointment so that we may properly check your insurance benefits and prepare for your visit.
Cancellation Policy
We ask that you please give notification of cancellation 24 hours prior to your scheduled appointment. Failure to do so may result in a $50 No Show Fee that must be paid prior to the scheduling of your next appointment. Please be aware that Monday appointments must be cancelled by noon on the previous Friday.
Late Policy
Being on time is very important to us. We understand circumstances happen that are beyond our control so we have implemented a policy to help run our office more smoothly.
The first time you are more than 15 minutes late for an appointment we will try our best to see you, but you may have to wait so that we can see patients that were on time for their appointment.
The second time you are more than 15 minutes late you may be asked to reschedule.
Payment Options
We accept most insurances and submit insurance claims. Our office financial policy is that all co-pays, co-insurances, and deductibles are collected at the time of service.
All cosmetic and laser procedure fees are collected in full at the time of service.
We accept MasterCard, Visa, Discover, American Express, and CareCredit. CareCredit is a medical credit card that is interest free for the first 6 months. You must purchase $500+ to be eligible for use. To apply, please visit CareCredit.com
Gift Certificates
Gift Certificates must be present at the time of service. Gift Certificates are non refundable and non transferable. Gift Certificates cannot be used for medical treatments. Revival Dermatology is not responsible for lost or stolen Gift Certificates. Any value not redeemed within two years of issuance will be deemed a monetary gift to Revival Dermatology.
Return Policy
Skin Care products (unopened) may be returned within 14 days for a full refund. Skin Care products (unopened) may be returned between 15-30 days for office credit ONLY. NO returns will be accepted after 30 days.
Treatment Packages are non refundable and non transferable.
